Passed away peacefully on February 13, 2013. Preceded in death by Husband, Luke. Survived by Daughter, Deborah (Jim) Carr; Grandchildren, Jeff (Maria) & Stephanie Carr; Sister, Joyce Allen (Special Friend, George Streich); Many Cousins, Nieces & Nephews. Mass of Christian Burial at 11AM on Monday, February 18, 2013 at Transfiguration Catholic Church, 6133 - 15th Street N. in Oakdale. Visitation one hour prior. Private Interment at a later date. Hazel's dedication to her husband and family was insurmountable.
