我們敬愛的父親史久炎先生於 二零一七年十一月六日辭逝於加拿大安大略省烈治文山市麥健時健康醫院

謹擇於二零一七年十一月十三日(星期一)設靈於多倫多市Jerrett Funeral Home上午十時接待、十一時舉行佛教誦經儀式

隨即出殯靈柩奉移多倫多市Forest Lawn Mausoleum and Cremation Centre火化，後擇吉安葬於多倫多市約克墓園

Peacefully passed away at MacKenzie Health with his family by his side on November 6, 2017. Loving father of Julisa, Anthony (Summer Kong), Wai Houng (Lan Yuen), Glendy (Andrew Chan) and Stephen (Karen Yep). Cherished grandpa of 9 grandchildren. Devoted great-grandpa of 1 great granddaughter. Kow Yim will be sadly missed and fondly remembered by his family and friends.

Friends may visit the Jerrett Funeral Home, 6191 Yonge Street (2 lights south of Steeles, on the east side) on Monday, November 13, 2017 from 10:00am to 11:00am. A funeral service will commence at 11:00am in the funeral home chapel. Cremation at Forest Lawn Crematorium to follow.
