MCINTOSH, Jack Lee, 85, of Surprise, Arizona, a United States Marine Corps Korean War "Chosen Frozen" Veteran, passed away on February14, 2018. He was preceded in death by his parents, John Lee McIntosh, and Gertrude McIntosh. He is survived by his son, Barry Lee (Jane) McIntosh; grandchildren, Keri Jane (R.C.) Castro, Brian Lee (Danielle) McIntosh, John Martin McIntosh, and Mandy Mae McIntosh; great-grandchildren, Amy McIntosh, Anna McIntosh, and Kayden Castro. A Committal Service with Military Honors will be held on Wednesday, April 18, 2018, at 11:00 AM, at the Prescott National Cemetery, 500 AZ-89, Prescott, Arizona.
