Andrew "Andy" Charles Azevedo, age 72, of Surprise, Arizona passed away on Sunday, November 21, 2021. Andrew was born September 25, 1949.
Andy is survived by his sister, Jolene (Dean) Michael; brother, Ron (Myrtha) Azevedo; and sister, Tina (Patrick) Councilor; and many nieces and nephews.
