Leonardo Dominick Chacon, age 4 months, of Phoenix, Arizona passed away on Tuesday, August 31, 2021. Leonardo was born April 15, 2021.
A visitation for Leonardo will be held Friday, September 17, 2021 from 1:00 PM to 3:00 PM at Advantage Funeral & Cremation Services – Maryvale, 6901 West Indian School Road, Phoenix, AZ 85033, followed by a funeral service from 3:00 PM to 4:00 PM.
