Jack P. Allen, 75, was born on June 14, 1937 in Barnesville, Minnesota to Amy and Harley Allen and passed away in Phoenix, Arizona on December 4, 2012. He leaves his beloved wife of 57 years, along with his daughter Deb and two sons, Jay and Jack, Jr. A Memorial Service will be held at Shadow Mountain Mortuary on Thursday, December 13, 2012 at 6:00 P.M. with visitation beginning at 5:30 P.M.
