Ronald Thomas Poljak, Sr. 69, passed away on April 6, 2018 in Palm Coast, FL. He was born in Pittsburgh, PA and served in the US Army during the Vietnam War. Mr. Poljak is survived by his wife of 47 years, Patricia J. Poljak; 2 sons: Ronald (Janice) Poljak Jr. and Nicholas (June) Poljak; 2 brothers: Edward (Kathleen) Poljak and Nicholas Poljak; sister, Charlotte (Charles) Koenig; 7 grandchildren: Daniel, Michael, Matthew, Benjamin, Jacob, Amelia and Eli. Family will receive friends 6-8 PM, Wednesday, April 11, 2018 with a Knights of Columbus service at HARDAGE-GIDDENS RIVERMEAD FUNERAL HOME, 127 Blanding Blvd, Orange Park, FL. Funeral Mass will be held 11 AM, Thursday, April 12, 2018 at St. Luke's Catholic Church, 1606 Blanding Blvd, Orange Park, FL with Father Andy Blaszkowski officiating. Burial will follow in Jacksonville National Cemetery with military honors. In lieu of flowers please make donations to St. Jude's Children's Hospital.
