Survived by her children & spouses: Dan Forby Jr. (Cindee), Cheri Peterson (Mark), Jay Forby (Tawney), Mary Forby (Scott), Tim Forby (Tammy). Grandchildren: Kelly Peterson Gilb (Andy), Matthew Ackerson (Tessa), Christopher Forby, Stephanie Forby, Thomas Forby, Andrew Forby & Great Grandson Lincoln; sisters, Diane Snyder (Tom), Jan Hungerford (Phil), Elaine LaSota (Denny) also many nieces, nephews and cousins, along with a world of friends.

Born and raised in Northeast Minneapolis, Carol was very proud to be 100% Polish! She graduated from St. Anthony of Padua High School. Longtime volunteer with Twin Cities Race for the Cure, Catholic ElderCare, St. Patrick's parish activities and the St. Hedwig's Perogi group.

Mass of Christian Burial 11:30 AM Tuesday, January 25, 2022 at the Church of St. Patrick's of Edina, MN. Visitation starting at 10:00 am.

Masks required at visitation and mass.

Memorials preferred to Catholic ElderCare.
