Frank Russell Polak of Lake Balboa, passed away on February 7, 2016, at 61 years old. He is survived by his mother, Jeanne Polak, sister, Sandi Wise, aunt, Penny Polak, nephews, Gregory, Adam, and Bradley Wise, and cousins, Robin Sampson and Randall Polak.
Services are being planned under the direction of Groman Eden Mortuary.
