Samuel Richard Olsen Jr passed away on February 8, 2015 in Mesa, AZ at the age of 76. Samuel was born on May 1, 1938 in Hamilton, OH and lived there until 1945 when he moved to Houston, TX with his family, and stayed there until 1948. He then moved to San Antonio, TX and attended Peacock Military Academy until 1950. Sam then moved to White Plains, NY and attended White Plains High School until graduating in 1956. Sam attended Rochester Institute of Technology from 1957 – 1960 in Rochester, NY and studied printing and photography. In 1960, he proudly served in the U. S. Marine Corps, and was transferred to San Diego, CA where he was stationed until 1963. He then moved back to Rochester to finish his schooling and earned his AAS Degree at the Rochester Institute of Technology. He then married his first wife Karen and together they welcomed two daughters Kristin and Erika. Samuel worked for Datagraphic as an Art Dept. Foreman from 1964 -1968 until promoted to General Manager in 1967. In 1968, he went on to start his own business Form Service Inc and in 1969 he welcomed his new son Samuel III into his growing family. In 1971 he formed a new company, Dealers Press Inc., on the back of his success with Form Service. Sam was elected to the Board of Directors of the National Business Forms Association and was awarded Manufacturer of the Year in 1971. He was reelected to the Board in 1973 for a two year term and was awarded his second Manufacturer of the Year in 1974. Later that year, he married his second wife Robin and in 1975 they welcomed their son Erik into the family. Also in 1975, his son Lonnie was adopted. In 1976, Form Service and Dealers Press merged to become one company and moved into a single facility in 1978. Sam resided in Barrington Hills, Illinois with his wife Robin for 10 years before retiring to Arizona where he resided until his passing. Sam was an avid fisherman throughout his life. Sam also enjoyed riding his Harley Davison Motor Cycles and also had a passion for dogs and competitive dog showing during his retirement years. Sam was also a passionate supporter of military veterans being a former Marine himself. A celebration of his life will be held at Mariposa Gardens, 6747 E. Broadway Rd in Mesa on Friday, February 13, 2015 at 2:00 pm.
