Hazel went on to marry the love of her life Clifford L. Phillips on January 15, 1969.
She was a long-time member of the Women of the Moose Chapter 477.
Her hobbies included bingo, playing cards and watching cops, but she was happiest when spending time with her family.
She is survived by her children, Timothy Phillips and Kristina (Keith) Meinert; grandchildren, Kathryn (Karl) Seger, Margaret (Tim) Phillips, and Nicole Phillips; several great grandchildren, 2 brothers and one sister.
She is predeceased by her husband, parents, 2 sisters, and 1 brother.
