He was born December 28, 1980 in Louisville, the son of Kenneth and Mary Theresa Smith.
Matt dedicated his career to working in senior healthcare.
Left to cherish his memory are his parents, Kenneth and Theresa Smith; his partner, Michael D. Collins; sister, Mary Jeanette Smith Thomas (Greg); two brothers, Kevin David Smith and Andrew Frederick Smith (Tara); niece and nephew, Stanley Kenneth Smith and Emma Lei Souza; his parents-in-law, Dale and Peggy Collins.
A service to celebrate Matt’s life will be conducted at 7:00 PM on Wednesday, June 28, 2017 in the chapel of Arch L. Heady-Cralle Funeral Home, 2428 Frankfort Ave. Family and friends may gather to visit from 1-7 PM on Wednesday at the funeral home.
