She was born on August 29, 1935 in Cave City, KY to the late Eugene Bush and Idella Logsdon. Hazel was a member of the Eastern Star, Parkland Baptist Church, Chicago Midwest Joint Board and she was the President of the Local 181.
Along with her parents, she is predeceased by her husband, Luther G. Roark. Here to carry on her memory are her daughters, Betty Casanova (Roy), Linda Sullivan (Darrell), Carolyn Jones (Bobby) and Barbara Shannon (Eric); grandchildren, Jerry and Jason Ritchey, Tami and Luther Sullivan, and Madison, Lucas and Molly Shannon; 6 great grandchildren; 3 great great grandchildren, along with her brother, Andrew Bush.
Visitation will take place from 2-8pm, with an Eastern Star Service beginning at 7pm on Monday, August 13th, at Arch L. Heady at Resthaven, 4400 Bardstown Road. A service celebrating Hazel’s life will be held at 12pm on Tuesday, August 14th at the funeral home, with a private burial taking place at Resthaven Memorial Park.
