Madison, David Wayne, 49, entered into eternal rest on November 22, 2015.
He was preceded in death by his brother, Brian Madison.
Left to cherish his memory is his daughter, Meredith Madison; soulmate, Katherine Block; mother, Jean Madison; father, James Madison; nieces, Candyce Block and Chanel Miranda; and many friends.
Visitation will be Wednesday, November 25, 2015 from 2 pm- 8 pm at Evergreen Funeral Home. Burial will be at Evergreen Cemetery.
Memorial contributions in memory of David can be made to the American Cancer Society.
