Sebastian was a dialysis technician with DaVita Kidney Care.
Sebastian is survived by his wife; Sarah Slick, daughter; Alyssa Wagner, sister; Lina (Brad) Johnson, brothers; Ryan Slick and Cody LaBreck, father; Jerry Ballinger, mother; Harumi Ikeda, mother-in-law; Lisa LaBreck, and nephews; Brady and Blake.
A memorial gathering for Sebastian will be held Sunday, April 13, 2025 from 1:00 PM to 3:00 PM at Family Funeral Care-Rockville West, 5791 Rockville Rd, Indianapolis, Indiana 46224, followed by a time of sharing at 3:00 PM.
In lieu of flowers contribution for Sebastian should by made to the American Cancer Society.
