Jacqueline was born March 23, 1957 to Juanita Remedies Ezernack and Monroe Ezernack.

Jacqueline is survived by; daughter Miranda Leonard and husband Dean; son David Mitchell; and step daughter Malonie Smith; sister Ruth Ann Bonneville, sister Mary Ann Lopez, sister Gail Hussey, sister Freda "Faye" Ezernack Stevens, sister Kaye Ezernack Stevens, twin brother Emmett Ezernack, brother Lee Ezernack, brother Kenny Ezernack, brother Steve Ezernack, brother Dennis Ezernack and sister-in-law Shirley Copeland.

Jacqueline was preceded in death by her husband William F. Smith; mother Juanita Remedies Ezernack and father Monroe Ezernack; brother Joseph Ezernack.

A visitation for Jacqueline will be held Tuesday, June 12, 2018 from 10:00 AM to 11:00 AM at Hill Crest Memorial Funeral Home Chapel, 601 US Highway 80, Haughton, LA 71037. A funeral service will occur Tuesday, June 12, 2018 at 11:00 AM, 601 US Highway 80, Haughton, Louisiana 71037. A burial will occur Tuesday, June 12, 2018 at 12:30 PM at Hill Crest Memorial Park and Cemetery, 601 US Hwy 80 East, Haughton, Louisiana 71037.
