He is preceded in death by his parents, brother Jack W. Lievsay, sister Ava Lee Grogan (Lievsay), Helen Payne (Lievsay) and Catherine Chumley.
Sam is survived by his wife Clara P. Lievsay, son Samuel F. Lievsay, daughter Savanaha, son Rickey, Jesse and 9 grandchildren and 7 great grandchildren.
