Dear Mrs. Forman & Family,

I was deeply saddened to hear of Dr. Forman’s passing. He was not just a colleague or mentor to me, but a friend whose presence had a profound impact on my life and career.

From the moment I began visiting St. Vincent in the early 2000s, Dr. Forman's guidance and support were invaluable. His experience, his hilarious sense of humor, and his blunt honesty were qualities that set him apart. But above all, it was his unwavering belief in me that truly made a difference. I'll never forget the way Dr. Forman convinced me that I would be successful.  

His ability to convey complex material with wit and humor made learning a joy. He was passionate about education and genuinely invested in the success of his students and residents.  He was a principled man who held strong to his convictions.

During my training at St. Vincent. Alongside Dr. Yu, he elevated our residency program to new heights, instilling in us a commitment to excellence that has stayed with me throughout my career.

Beyond his professional accomplishments, Dr. Forman was a true mensch. His kindness, caring nature, and willingness to help others were evident to all who knew him. His jokes and humorous stories about life and practice, brought laughter and joy to those around him.

While we mourn the loss of a dear friend, colleague, and mentor, we also celebrate the incredible legacy that Dr. Forman leaves behind. His impact on the podiatric community and the countless lives of patients he touched will be remembered for decades to come.

Mrs. Forman, please accept my heartfelt condolences during this challenging season. Know that Dr. Forman’s memory will live on in the hearts of all who had the privilege of knowing him.

My sincerest condolences. Dr. Foreman was a wonderful human being and doctor. He was my favorite one to see and I looked forward to my appointments, despite the pain I would be in. He helped me immensely and I will forever be grateful. He left a lasting impact on me as a patient. Sending love and light to all of you.

I can only echo the beautiful words of my former Chief Resident Jeff Lynn. Mike was one of the great guys in our profession. He was a huge advocate for me when applying to the Mount Sinai Residency in 1991. I shadowed him prior to the residency match, watching him perform surgeries at Deaconess Hospital and in his Beachwood office. When asking him who the residency was picking, knowing he really couldn't say outright, his reassuring comment was "Jason, you've got nothing to worry about". He was a comforting source during the rigors of the following two years of training. "He would give you the shirt off his back" was not an overstatement when it came to Mike. Years later, when I was involved in very serious personal issues, he again was there for me, offering any help I needed. I just found out about his passing, and I'm very sad today. Besides my wife and I, he was third person to hold my daughter Laura after her birth at Sinai. The 4th was Gerard Yu, whom I also miss very much. Much love to Blanche and the whole family. May the many memories of Mike always be for a blessing.

Dear Blanche and family,
I was so very saddened to hear of Michael’s passing.  Michael was my mentor, my teacher and later became my colleague and dear friend.  As a professor, Dr. Forman taught my favorite biomechanics class in school.  
He always had a knack for conveying the material in a way that we as students could easily relate.  He used his wit and sense of humor to make an otherwise dry material fun and entertaining.  Michael genuinely wanted his students to learn and succeed.  He was a passionate educator.  
Dr Forman was chief of the department of podiatry when I was a resident at Mt Sinai Medical Center.  
He worked closely with Dr. Gerard Yu, the residency director, and together they elevated the residency program to elite status.  Michael was affectionately known as “chief” to all the residents that were lucky enough to have trained under his guidance. 
Michael was extremely kind, caring and always willing to help a colleague, friend or anyone in his life.  He had a positive outlook and took a vested interest in others.  
I will miss his jokes and many humorous stories enhanced with his patented accents.  
I cherish the fond memories I have of Michael over many years.
He was admired and respected by so many friends and colleagues.  Michael was completely authentic and genuine.  
Of course he was most proud of his loving family.  
We lost a very dear man but his legacy will live on forever in our hearts.  
Rest in peace “chief”, you’ll be dearly missed.

Dr. Forman, or Michael as he insisted for years that I call him, was such a gift.  As the father of my friend Amy, he always made sure that I (and eventually my husband and children) felt part of the family.  He would say that we were always welcome and genuinely mean it.   He was genuine with everything.  Spending time at the lake house with him and Blanche are memories that my entire family cherishes.  He remains a  kind, funny, warm spirit who will be remembered and missed by so many.

We were so sorry to learn of Michael’s passing yesterday.  We have only the best memories of him from when we were first year students at O.C.P.M.   He had a wonderful sense of humor and was a great teacher.  We were blessed to be friends for the past 40 years.  Sending our heartfelt condolences to you and your entire family with love.

Dear Blanche,
Lillie Ann and I are so saddened about Michael's passing.  Indeed, I considered him a wonderful friend, and I wish we had had more time together.  You and Michael were so good and so kind to us, and we will treasure forever our friendship with you and Michael.

My heartfelt sympathy for Blanche, Joel and Amy.  The Lord has received a very fine gentleman and a wonderful doctor. Mike was a fine caregiver as a podiatric physician. He was a gifted biomechanics instructor and inspired many young doctors.  Mike had a great sense of humor and enjoyed his friends. It was an honor and a privilege to have him as a friend and colleague!

So sorry to hear of Michael’s passing. After Covid started, we weren’t able to keep up our traditional weekend breakfast at Corkys. Even after Covid was under control, we never did resume our weekly meetings. Michael’s stories and jokes will be missed. He was a great friend.

Mike,

Almost 40 years ago I was a pre-med student, not sure of what direction I wanted to go in. Dad said, “Go see what Mike does.”  That day in your office was a defining moment in my life, the first step toward a long and rewarding career as a podiatrist myself.   Your sense of humor got me through Biomechanics class (I still talk about Alex Valgus) and you showed me that performing surgery can actually be fun!  I was so proud to have my name next to yours on the research study we worked on together, and the published article is still displayed in my office. 

More than just a family friend, you were my inspiration, my professor, research partner, mentor and friend.   You have impacted my life in so many ways, Michael, and I will forever be thankful.

My heartfelt condolences to Blanch and family.
Mike Forman was not only my mentor and teacher but also a very good friend and colleague. Hearing of his passing was truly heartbreaking. Mike was a teacher, leader, an advisor and a confident to all. His humor was a gem. Mike told a seminar group that while playing golf with him I drove a golfball 420 yards. He failed to mention it was a slice, downhill and hit the cart path three times. I like others cherish these jesters. 
He touched many of us with his friendship and  love.  I loved Mike and will miss him.  Elliott Biggs

I was so sorry to hear of Michaels’s passing. My heart goes out to all of you and I know he will remain in your hearts always. I know he had a difficult few years toward the end of his life but he also had many good years with you, Blanche, and his children and grandchildren. Michael was a one off and will be remembered with a fond smile by all who knew him.
