Samuel Phillip Cook, age 23, died at 1:43 a.m. on Wednesday, June 7, 2017 at OSF St. Francis Medical Center in Peoria. He was born on August 1, 1993 in Bloomington, the son of Patrick and Rose Anne Standaert Cook.
Sam worked as a meat wrapper at Jewel-Osco in Bloomington for two years, and most recently worked as an inventory specialist at Illinois Voices Theatre. Sam was an active member of the Salvation Army Church in Bloomington and was a member of the Bloomington Center for the Performing Arts.
Sam is survived by his parents of Bloomington; a brother, Stephen Cook, currently serving in the U.S. Marine Corps; two sisters, Anne Cook of Springfield, and Bailey Cook of Bloomington; maternal grandparents Ray Standaert and Marty Hamilton of Bloomington; as well as several aunts, uncles, and cousins. He is preceded in death by his paternal grandparents, Virgil and Betty Cook.
A celebration of life will be held on Tuesday, June 13th at 10:30 a.m. at the Salvation Army, 611 West Washington, Bloomington with Captain Paul James officiating.
In lieu of flowers, contributions may be made to the Salvation Army or the Bloomington Center for the Performing Arts.
