Donna A. Sladek 67 , Born November 22 ,1953 Tucson Arizona at St marys hospital passed away November 1st ,2021 in Phoenix Arizona. She is survived by Only living Son Jerry Thompson , Daughter in-Law , and two granddaughter's Natalie, and Madisyn and grand-pup Eli, and her brother Dave Sladek . She loved going on trips with the family , she enjoyed crocheting , swimming, board games and videogames with with grandchildren and playing with grandpup Eli and getting love .She enjoyed many years with her kids and grandchildren trips and dinners and hoildays . She worked for the state Dept of Health Services Medical Facilities for 36 years .She will be missed more then words can explain and will be never forgotten . REST IN LOVE DONNA.
