Our hearts are broken with the sudden loss of our daughter, sister, mother and friend, Amy Lynn Whitaker, 33, who passed on February 12, 2017.
Amy was born November 22, 1983, the daughter of Mark and Janna Forsgren. She graduated from Highland High School in 2002 and worked as a medical Assistant at multiple medical clinics in Salt Lake City.
Amy was the mother to Hayley Lynn Whitaker, age 10. Her daughter was the light of her life, every moment spent and memory with her are the ones that she treasured the most. Amy was loved and adored by her family and many friends.
Survived by her mother Janna Forsgren, sister Mandy Forsgren and daughter Hayley Whitaker. Preceded in death by her father Mark Forsgren, grandparents Starling Bryant, Ferron and Elizabeth Forsgren.
A Memorial Service will be held in her honor at Wasatch Lawn Mortuary on Monday, February 20 at 11:00 am, 3401 South Highland Drive, Salt Lake City, Utah 84106.
