He was born to the late Kenneth and Margaret Chism on April 3, 1941 in Louisville.
Tom loved playing golf, being a member at the Oldham County Country Club for over 40 years. He also enjoyed playing poker and fishing. Tom was an avid UK fan and lived "Go Big Blue!"
He was preceded in death by his step-daughter; Diana Wigginton.
Left to cherish the memory of Tom are his wife of 27 years, Shirley Chism; son, Michael Chism (Crystal); step-daughters, Donna Smith and Robyn Gordon Rakestraw; 8 grandchildren, Roman, Max, Archer, Kristin, Lauren, Natalie, Spencer and Ashley; 2 great-granddaughters, 2 sisters, Kay Human (Andy) and Patti Dew; dear friends, Linda and Tony Osting, extended family and friends.
A celebration of Tom's life will be held on Monday, July 31, 2023 at Heady-Radcliffe Funeral Home, 706 W Jefferson St, LaGrange at 6pm with visitation from 2pm until the start of the service.
Memorial donations can be directed in Tom's name to Southern High School Class of 1959 Scholarship Fund in Louisville.
With Tom's passing there's a three-some at the Teebox looking for a fourth, a card game short a player, a large mouth bass that will live another day and the CATS are short one fan!
