I am so sorry about the loss, your loss of Doctor Cook. He was a great man , good friend and respected colleague. Many patients are saddened by his passing! They too respected  and considered him a great man.
My deepest, sincere condolences to you and your family! He is missed. Multicare will never be the same without him!

My acquaintance with Dr. Cook has been a mere 3-4 year old one . But he had a major impact on  my professional life . He was the most funny person in the clinic . I still can’t believe I am addressing him in the past tense . We miss Dr.Cook . 
Patients and staff loved him .
His love for fishing and hunting was so obvious and he entertained us with his stories and pictures of the stuff he caught over the week.
He loved his wife a lot and that was very obvious in his conversations and he mentioned “ coco” in his talk almost everyday . 
I would like to send my love and prayers to the family and may the good lord comfort them . 


My condolences to the family of a wonderful man that has left me with great memories and plenty of laugher may he rest in peace, god bless.

Kollette my dear friend...I knew when you met Jeff he was your soul mate. He was huge in life. When Jeff is around...you knew it. He was loud and obnoxious at times...but thats why we loved him...he lived HIS way and never strayed from it. We have so many great times...darts and bags...cruising the lake. Our friendship seen highs.and lows...but I never would trade our memories for anything. I only wish we had more time with out friend Jeff...a husband, a dad, a son, a grandpa, an amazing friend who will be missed. We love our Jeff and Kollette. 

One of our purpose here on earth is to be a positive vessel to one another, and Dr. Cook displayed that action in every aspect of life. Each day that Dr. Cook came into work his presence was know, Dr. Cook lit up the room with his smile & his voice. I will truly miss my morning greeting, “good morning sunshine “.  Good morning sunshine will forever be stamped in my heart…
