Maxine Mae (Koubek) Hyatt was born in Carlinville, Illinois to Wesley and Clara Koubek passed peacefully at home in Phoenix, Arizona. She is preceded in death by her parents, daughter, Lisa Elizabeth, and son-in-law Christopher Fitzmaurice. She is survived by her husband Harley Ray, daughter Cathy and her husband Jack Johnson, and daughter Kerry Fitzmaurice; along with grandchildren Carissa and Jacob Johnson, Grace, Taylor, Bailey and Payton Fitzmaurice. Maxine will be remembered for her witty sense of humor, passion for cooking and feeding others, and her love for animals. A celebration of Maxine's life will be held on October 22nd.
