Carlo De Simone was born in Medford, Massachusetts on June 2, 1967 to parents Mario and Johnna De Simone. He was the second child of four, with an older brother and two younger sisters. He was raised in Massachusetts until the age of 13 when the family relocated to Phoenix, AZ.
Between the age of 28 and 37, Carlo had three daughters who he adored and protected fiercely. In 2009, Carlo relocated his family to Iowa where he attended the nursing program at SWCC, graduating with an ADN in 2012. The family moved back to Phoenix after graduation to be closer to his parents, where he worked as a nurse in various specialities including skilled rehabilitation, hospice care, dialysis, and addiction treatment. Carlo touched the hearts of all who had the opportunity to meet him. Carlo De Simone passed away at the age of 56 on October 29, 2023 in Glendale, AZ. His kindness and warmth live on in his loved ones. Carlo is remembered by daughters Giuliana, Isabella, and Ava; his siblings Mario, Jennifer, and Marissa; his parents Mario and Johnna; as well as all the friends, coworkers, and patients he touched!

A visitation & gathering for Carlo will be held Monday, November 13, 2023 from 4:00 PM to 8:00 PM at Shadow Mountain Mortuary, 2350 East Greenway Road, Phoenix, AZ 85022. A funeral mass will occur Tuesday, November 14, 2023 from 10:00 AM to 11:00 AM at St James Roman Catholic Parish, 19640 N 35th Ave, Glendale, AZ 85308. A committal service will occur Tuesday, November 14, 2023 from 11:45 AM to 12:30 PM at Phoenix Memorial Park and Mortuary, 200 West Beardsley Road, Phoenix, AZ 85027.
