Jack Lewis Cook was born June 26, 1933, and died March 10, 2011. He retired from the Department of Human Services, Architectural and Engineering Unit as an inspector for new construction at Children’s Hospital and O’Donoghue Rehabilitation Institute. He was preceded in death by his father, Jerry Lee Cook; mother, Martha Koeltzow Cook; one older sister Betty Robinson of Midwest City, two older brothers, Vinson Cook and Jerry Paul Cook. He is survived by spouse, Jacqueline Bennett Cook of Oklahoma City, brother Fred Cook of Carrollton , Texas; two sons, Jerry Martin and wife Irene Cook of San Antonio, Texas and Jack Cook, Jr. and wife Priscilla of Yukon; a daughter, Kay and husband Bryan Dickinson of Oklahoma City; four grandsons, two granddaughters; one great grandson; and two great granddaughters. Jacqueline Cook expresses gratitude to the staff of Easter Seals Adult Day Health Center, numerous physicians and staff of OU Physicians, particularly those practicing in the Department of Family and Preventive Medicine at OU Health Sciences Center for their competent, compassionate care during his final years of life, and to the staff of Ranchwood Nursing Center of Yukon where he spent his final days.
