Jack is survived by his wife, Debbie Mentz; children, Sharon Davids (Jason), Lisa Elcock (Ron); grandchildren, Kevan, Trevor, and Jaida Elcock, Rhiannon, and Grant Davids; great-grandchildren, Keelon and Cruz Elcock; siblings, Julie, Jim, Jerry.
Jack was born on January 27, 1942 in Oak Park, IL to John and Jerree Mentz. Jack graduated from Carroll College with a degree in Biology. He married Debbie in 1967.
Jack retired as a plumber. He was active in Boy Scouts and Square Dancing.
A funeral is scheduled for 10 am on Saturday, September 24 at Mariposa Gardens in Mesa, AZ, with a reception to immediately follow. All are welcome to attend and celebrate Jack’s life. In lieu of flowers, please send donations to any Parkinson’s foundation (Parkinson’s Foundation, Michael J. Fox Foundation, Muhammad Ali Center, American Parkinson Disease Association, Neuro Wellness Charitable Organization) The family would like to thank the caregivers at American Orchards and the Banner Neuro Wellness Center for their dedication to his well-being.
