David Wayne Cook, 54, passed away December 23, 2019.

David is preceded in death by his parents, Delphia Woosley Cook and Lucin Cook; and brother, Ronnie Cook.

Left to cherish his memory is his loving wife of 15 years, Kim Cook; children, David Shears (Haley), Ciarra Cook Barrett (Richard), Richard Allen (Ruby), Lamanda New (Chris), Brittany Southard (James), Amber Hardin (Allen); 16 grandchildren; sisters, Patricia Morgan (Darrell), Betty Brewer (Willie); and 3 fur grandbabies.

The Funeral Service will be Monday, December 30, 2019 at 2:00pm at Evergreen Funeral Home with visitation on Sunday from 11:00am – 8:00pm at the funeral home.
