Mohammad Azam Tabib, M.D., formerly of Valparaiso, IN, passed away on March 20, 2017. Dr. Tabib was a devoted and loving husband, father, son, and brother; a respected surgeon; and a loyal mentor and friend. He is survived by his wife of 36 years, Dr. Virginia Tan Tabib of Chicago, Illinois; his three children, Grace Hedyeh Tabib (Geoffrey Reed) of Los Angeles, CA, Daniel Hadi Tabib (Zachary Fox) of Sao Paulo, Brazil, and Dr. Christian Hooman Tabib of New York City, NY; his two grandchildren, Elijah and Talia Reed; his mother, Akram Morid; his brother, Mohammad Arfa Tabib; his four sisters, Aghdas, Shirin, Shadokht, and Dr. Mahasti Tabib; and loving uncles, aunts, nieces, nephews, and cousins. Dr. Tabib was born on March 21, 1949, to Akram Morid and his late father, Dr. Abdol Hadi Tabib in Shushtar, Iran. After completing his medical degree at the University of Tehran, he immigrated to the United States at the age of 26 to pursue a career in medicine. He completed his surgical urology residency at the University of Illinois, Chicago, and his pediatric urology fellowship at Children’s Memorial Hospital, Chicago. It was during his residency when he met his wife of 36 years, an internal medicine resident, Dr. Virginia Tan Tabib. Dr. Tabib spent the next 30 years living in Valparaiso, Indiana, where he served patients throughout Northwest Indiana, including Porter Health Care System and La Porte Hospital, and impacted thousands of lives as a practicing urologist. Dr. Tabib devoted his life to his family and his patients. He enjoyed traveling the world; spending quality time with his family and friends; conversing about international affairs; and doing everything in his power to ensure those around him were happy. A memorial service celebrating Dr. Tabib’s life and legacy will be held at Pierce Brothers Westwood Village Memorial Park and Mortuary, Los Angeles, CA, on Saturday, April 1, 2017. The viewing will be held from 2-3 pm followed by a memorial service.
