She is preceded in death by her parents, Albert and Maggie Louise Couch.
Hazel is survived by her son, Johnny Price of Riverdale, as well as a host of other family members and friends.
Hazel will be laid to rest on Saturday, April 16, 2020 at Sherwood Memorial Park - 6841 Tara Blvd. - Jonesboro, GA 30236.
