Jack was born January 10, 1934, in Port Jervis, New York, to the late James and Ida (McGrath) McCormack. Jack married Hazel (Collins) on December 11, 1954, in Port Jervis and later moved to Gladstone, Missouri. Jack was an amazing Husband, Father and Papa.
Jack is survived by his daughter, Susan; four grandchildren, Isabella, her husband, Andrew, & their daughter, Cora; Christa; Shawn his wife, Rebecca, and Kara & her daughters, Nevaeh and Lilly; as well as three grand dogs, Tipsy, Hazy, and Stanley. Jack also survived by his niece, Donna Schanzenbacher, her husband, Peter, & sons, Ryan and Eric; and two sister-in-laws, Betty Collins and Arlene Cavanaugh.
In addition to his parents, Jack was preceded in death by his wife, Hazel; son, Michael; and sister, Linda Smith.
Inurnment will be at White Chapel Cemetery, Gladstone, Missouri.
