

D’Amato, Salvatore “Sammy” Born to Life May 23, 1931. Born to Eternal Life December 28, 2011 at the age of 80 years. Preceded in death by his parents Carmello and Antoinette, sisters Mary and Francis, and brothers Frank, Joe, Casper and John. Survived by brother Carl (Theresa) D’Amato and 4 generations of nieces and nephews as well as other relatives and friends.
Sam’s family extends a special “Thank You” to the staff at Community Memorial Hospital for the care and sincere compassion shared with all of us during the last few days of Sam’s life. The Comfort Care Program, their ministry and team work among the nursing staff will not be forgotten. We thank you and Dr. Halstrom for their excellent care.
We also extend our gratitude to the Care Providers who provided Sam with the utmost respect and care. Sam had a wonderful life. Sam’s primary care provider, Kris, worked with Sam for 17 years, maintained consistent staffers, Joe for 16 years and Ernestine for 10 years. Sam was a “happy guy” always ready to be on the go – Brewer Games, Green Bay Packer Games, activities offered within the community. He loved listening to the music at Festa Italiana and Polish Fest. Sam and Kenny, his roommate for 12 years, went on yearly vacations. “Ma” D’Amato would be so proud to know how happy Sam was because his life was so much fun. Sam vacationed in many places such as: New York, Niagara Falls, Branson, Disney World, and the Wisconsin Dells. Joe took Sam fishing, Kris got Sam into a bowling league. Sam continued to bowl with Kenny – gutter ball, gutter ball, gutter ball – but what fun they had! Sam hosted a yearly Halloween Party, and loved planning “his” Birthday Parties inviting several group homes to attend and play POLKA MUSIC. Sam spent his days at Alexian Village participating in an abundance of activities. We thank Alexian Village and their staff for their excellent loving care. Sam had a lot of friends and was everybody’s sweetheart. We loved him so – our great memories of special moments with Sam will remain with us forever.
Visitation Tuesday, January 3, 2012 at Blessed Savior Catholic Church (8545 W Villard Ave. Milwaukee) from 9:30AM until time of Mass. Mass of Christian Burial at Church 11 AM. Procession to Holy Cross Cemetery following Mass.
