Patricia " Pat " Joan Hayes, 72 of Surprise, and formerly of Kansas City, MO passed away on June 8, 2012 with her family near her side. Pat was born on September 12, 1939 in Milwaukee, WI to Harry and Lillian Ozminkowski. Pat was an RN; she cared for many and especially loved caring for her family. She will be deeply missed and loved by those close to her. Pat is survived by her beloved husband of 51 years, Thomas C. Hayes; her two loving daughters Becky (Kirk) Miller and Cindy (Kirk) Mitchell; six loving grandchildren Courtney, Ian, Sarah, Katie, Charley and Tom; one brother Mike Manske. The Memorial Service will be Monday, June 18, 2012 at 1:00 PM, with Visitation from 12-1 PM at Surprise Funeral Care, 16063 W. Bell Rd., Surprise, AZ.
