He was born to the late Cecil and Vera Brown Hazlett in Norton, Kansas on August 7, 1955.
Phil's greatest joy in life was spending time with his grandchildren and making positive memories with them. He was proud of the impact he had on others through his sponsorships with NA and AA.
He is preceded in death by his parents, brother; Ricky Hazlett and beloved dog; Mandy.
Left to cherish the memory of Phil are his children; Shawn Hazlett (Stephanie), Brian Hazlett (Erica) and Amanda Curtsinger (Patrick), 8 grandchildren; Harley, Ariston, Phillip, Sidney, Juliet, Rosie, Clara and Reagan, siblings; Ron Hazlett (Ginger), Wayne Hazlett and Elaine Lamont (Dennis), mother of his children; Marie Rigsby, former wife; Pam Lowry, step-daughter; Julie Lopes, extended family and friends.
A celebration of life will be held for Phil at the John W. Black Community Center, 1551 KY-393, LaGrange, KY 40031, on Saturday, June 11, 2022 from 2-4pm.
