He was preceded in death by both parents.
Thomas enlisted in the United States Army in 1966, having served in Germany, as well as having served a tour in Vietnam from 1968 - 1969 as a gunner. He returned home on June 20, 1969. He married his wife Janice on August 17, 1969, and they recently celebrated 54 years of marriage. He was an avid hunter and fisherman, and loved to spend time in his garden. Thomas also enjoyed traveling and playing cards. He retired from Dayton Superior in 2010 after 40 years of service. He is a member of the Franklin First Church of God, where the Children's Church building is named after his mother, Faye Hazlett.
Tom is survived by his wife Janice Hazlett; daughters, Heidi Siens (Kurt), and Heather Faulkner (Jason); grandchildren, Kyle and Blake Fella, Mackenzie and Olivia Faulkner; great grandchildren, Joel and Lily; sisters, Linda Miller (Tom) of Franklin, and Carol McNally (William) of Orlando, Florida; along with numerous nieces and nephews that he loved as his own. He is also survived by many lifelong friends and neighbors.
A visitation for Tom will be held on Friday, November 3, 2023 from 10:00 am - 12:00 pm at Anderson Funeral Home, 1357 E. Second Street, Franklin, Ohio 45005. Funeral service will immediately follow at 12:00 pm with Pastor Mark Crider officiating. Burial will take place at Springboro Cemetery.
In lieu of flowers the Hazlett family has requested that memorial donations in Tom's name go to https://agentorangerecord.com/donate/ to aid with veterans and their families that were exposed to Agent Orange.
