Terry Blake, 84 of Ft. Smith passed away Wednesday, September 16, 2015 in Bryant, AR. She was born January 13, 1931 in Hopper, AR. She was a member of Glad Tidings Church where she was an Usher and Sang in the Choir. She was a retired Home Health Nurse. She was preceded in death by two sons; Danny Wise and Kenney Sprute.
Graveside services will be 1:00 PM Monday, September 21, 2015 at the Sallisaw City Cemetery in Sallisaw, OK. Arrangements are under the direction of Fentress Mortuary in Ft. Smith.
Ms. Blake is survived by three sons; Tom Jaskowiak and Timmy Jaskowiak both of Duluth, MN and Terry Jaskowiak and his wife Gayle of Bryant, AR. She is also survived by six grandchildren; Curtis Wise, Stephanie Jaskowiak, Jason and Brandon Wilson, Kelly and Jennifer Wise as well as five great grandchildren.
Pallbearers will be Jason and Brandon Wilson and Terry Jaskowiak.
Visitation will be 2:00 PM – 4:00 PM Sunday September 20, 2015 at the funeral home.
