A visitation for Sam will be held Sunday, September 29, 2024 from 3:00 PM to 8:00 PM at Woodlawn Funeral Home, 7750 West Cermak Road, Forest Park. Family and friends will meet Monday September 30 for the Mass of Christian Burial at Ascension Catholic Church, 808 S East Ave, Oak Park, at 10:00 AM. Interment will be held privately.
All are encouraged to fondly remember Sam by proudly wearing orange and blue.
