I am so sorry for the loss of my cousin Jerry. There are so many fond family memories that I have and all include him. May he rest in peace and may all his family and friends cherish the memories they have of him. He will be missed by many.

A man who could keep an entire room entertained for hours with his stories from his own experiences he has definitely lived more than a life so lets continue those stories and may these memories keep us at peace during this loss.

This is one of the hardest things I've ever had to do. Words cannot describe the grief and loss that we are feeling, as we gather today January 2nd, to say farewell to our son, nephew, big brother, uncle, father and friend Jerry, Born October 31, 1967 and left us on December 23rd 2020 but I will try. He leaves a huge emptiness in the hearts of all of us who loved him, many who were his friends, and even more who just had a glimmer of him through our family, his painting, and neighbors he introduced himself too.
Jerry was a wonderful, sweet, loving person who never met a stranger. Jerry loved his family, tattoos which he told me think of it as body art, was an amazing painter who always had a story to share.
He loved his memories of camping at Fish Lake Beach with his Grandma and Papa, being a son, nephew, big brother, uncle, and father.  He was so looking forward to being a Great Uncle to his niece Alexis and Matt’s new arrival. I’m sure there will be many adventures to come that have yet to be shared with me as times passes.
Death can’t separate people from love. It can’t take away our memories either. In the end love is a precious gift shared with us through God.
To have lost Jerry is heartbreaking – it has come as such a shock to us all. His life was far too brief.
My family wishes to express our heartfelt thanks to all those who have given their support, compassion and love throughout this very difficult time. I know in my heart that he would not want us to be grieving for too long. Rather, Jerry would want us all to remember the good times we all shared with him.
Farewell, our son. nephew, big brother, father, uncle, and friend. You will live in our hearts and memories forever.
May you rest in peace; we will miss you more then you could possible know.

Jerry was always my favorite cousin. As I kid I remember him coming out to my house every chance that he had. He said he loved to get out of the city. Every time he came he brought his jug full of city water because he could not stand the taste of well water. Jerry would always defend me when my brother was being mean to me and he always showed me what a caring person he was. As teenager's all of my friends wanted to date Jerry. He was always so good looking. As adults we may have not talked to each other often but when we did he always had plenty of story's to share with me. Jerry loved his papa (buddy) and his grandma (Alice) and I know he is so happy to be reunited with them. Rest in peace Jerry, until we meet again, love your cousin Kim

I have a lot of memories of Jerry. He loved to come to my house as a kid and play with his cousins Patrick and Kim. I was so happy to see him at the last family reunion. I know he is in Heaven and welcomed by Grandma Alice and PaPa Buddy

I remember him always being around when his Aunt and I would hang out together. One day we were sitting on the front stairs of the house and he told me he was going to marry me. I had to explain that couldn't happen and he was so disappointed. He was such a happy beautiful boy. I'm sure he will be missed but happy he will get to see his grandma and grandfather up in heaven.

I am so very sorry to hear of Jerry’s passing.  Please accept my deepest sympathies to the Blazek family.
